証明書が旧姓等で、現在の名前と一致しないのですが、そのまま提出してよいですか。
My certificates are issued with my maiden name and does not match with my current name. Can I submit them as they are?

　御本人か確認する必要があるので、証明書の名前と志願票等の名前が同一人物である証明書をご提出ください。

　Please provide with the evidence which proves that the applicant and the holder of the certificate are the same person.

　※ほとんどの場合は、旧名と現在の氏名の記載が両方ともある｛戸籍謄本、戸籍抄本、受理証明書、(条件次第で)住民票の写し｝などで証明をします。お住まいの市区町村役場にお問い合わせください。

   ※二重国籍であることでそのようになる場合は、両国のパスポートの｛顔写真および氏名等の記載があるページ｝の写しをご提出ください。

　※その他、現在通っている教育機関などの公的機関において、同一人物である証明書を作成してもらえるようであれば、それでも構いません。但し、原本として処理ができるものを必ずご確認ください。

　※In most cases this can be confirmed by Certificate of the Family Register, Certificate of Individual Records or Certificate of acceptance which has one’s former and current names. A certified copy of resident register will do depending on the situation. Please inquire your local city municipality.

※If there is such a case due to possessing a dual citizenship, provide a copy of both passports ｛The page with a photo｝.

※If you are currently studying at an institute and if they can provide you with a certificate which can prove that your name has changed after marriage that is also fine.
Make sure the certificate should be original.
