Annual medical checkup 2014 (2014/6/4-5)

Dear all,
We would like to inform that the annual medical checkup will be held on June 4th and 5th, 2014. Please find attached the information for details.
All of you need to come to OFIAS during May 27 (Tue) - June 3 (Tue) to receive the necessary documents and materials for checkup (those of you who plan to have the optional stool test for occult blood, please receive the materials by June 2nd).
