[Lecturers Only] Monthly working report for 2014

Dear all,
Please find attached the monthly working report forms for 2014 (for April through December). Please fill out the form about your working state by checking the applicable box(es) and submit it by every 6th of the following month.
(For new professors)
All the TUFS faculty members (including visiting professors) need to fill in the monthly working report form every month in order for us to grasp the working state of faculty members.
Please fill out the form about your working state by checking the applicable box(es). (Please refer to the example sheet included.) This monthly work report must be signed and submitted to the Administrative Office (Room 301) of the Research and Lecture Bldg. by every 6th of the following month.
For Japanese-speaking professors, the form could be downloaded from the website as well.
